Bilingual Behavioral Health Clinician II – Integrated Primary Care

Wisconsin | Full-Time (Monday-Thursday 7:00AM - 5:00PM)

Noble Community Clinics is seeking a Bilingual Behavioral Health Clinician II (Spanish/English preferred) to provide integrated behavioral health services within our Primary Care Behavioral Health (PCBH) model. In this role, you will work side-by-side with primary care providers, nurses, and care teams to deliver brief, evidence-based interventions that improve both behavioral and physical health outcomes.

What You’ll Do

  • Provide same-day (“warm handoff”) behavioral health consultations in primary care.
  • Conduct focused behavioral health assessments and risk assessments.
  • Deliver brief, solution-focused, evidence-based interventions.
  • Support patients with concerns such as depression, anxiety, stress, substance use, sleep issues, chronic pain, and chronic medical conditions.
  • Collaborate closely with primary care providers and interdisciplinary care teams.
  • Participate in daily huddles, care planning, and consultative team meetings.
  • Coordinate care with community agencies and connect patients to needed resources, including housing, transportation, food security, and other enabling services.
  • Document assessments, treatment plans, and care coordination activities in the EHR.

What We’re Looking For

Required Qualifications

  • Master’s degree in Social Work, Counseling, Psychology, Marriage & Family Therapy, or related behavioral health field.
  • Current Wisconsin licensure as an LCSW, LPC, or LMFT.
  • Bilingual in Spanish and English
  • Basic Life Support (BLS) Certification (or ability to obtain within the required timeframe).
  • Experience providing assessment, diagnosis, counseling, and psychotherapy services.
  • Strong collaboration, communication, and organizational skills.
  • Commitment to patient-centered, integrated care.

Preferred Qualifications

  • Experience in integrated primary care, community health, FQHC, or PCBH settings.
  • Familiarity with PHQ-9, GAD-7, SBIRT, and other behavioral health screening tools.
