Position Summary
Candidates must be located in or near Palm Bay Florida


This role will require 50-75% travel for face-to-face visits with members in Brevard county FL


Schedule is Monday-Friday, Standard Business Hours 8am to 5pm.


We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Case Management Coordinator to join our Case Management team. This opportunity offers a competitive salary and full benefits. Case Management Coordinator is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness. Case Management Coordinator will effectively manage a caseload that includes supportive and medically complex members. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration. Case Management Coordinators will determine appropriate services and supports due to member's health needs; including but not limited to: prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, community resources and supports.  


Required Qualifications

- Bilingual in English & Spanish

- Must reside in or near Palm Bay FL

- Must possess reliable transportation and be willing and able to travel up to 50-75% of the time to meet members face to face in Melbourne and surrounding areas Palm Bay and Satellite Beach. Mileage is reimbursed per our company expense reimbursement policy
- 1 + year of experience in behavioral health or a long-term care setting

- 1+ year of experience in case management and discharge planning experience

- Computer proficiency in Microsoft Word, Excel, and Outlook required


Preferred Qualifications

- Ability to multitask, prioritize and effectively
adapt to a fast-paced changing environment
- Effective communication skills, both verbal
and written
- Managed care experience

Education

- Bachelor's Degree required, preferably in Social Work or related field

Pay Range

The typical pay range for this role is:

$21.10 - $40.90

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
