Qualifications & DutiesESSENTIAL JOB FUNCTIONS:


Coordinate the assessment and delivery of grief counseling needs and services (one- on-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers
Plan for Bereavement Program development, expansion and refinement through annual evaluation of services
Provide bereavement support to hospice personnel coping with work related grief through one-on-one counseling, supervision of interns providing one-on-one counseling, and referral to community resources
Provide bereavement information and referral services to callers from the community
Expand and maintain community resource list, as needed
Assist as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats, weekly support groups)
Function independently but seeks and accepts guidance from other members of the interdisciplinary group, or from members of the community
Participate in hospice activities such as in-service education, hospice personnel meetings and relevant committees
Advance professional knowledge and skills


REQUIRED EDUCATION & EXPERIENCE:


Must possess a bachelor’s degree or equivalent specialized experience or theological degree from a college or university – OR – master’s degree in social work with qualifications of experienced clinical grief counseling
Must have experience with families/caregivers and patients through death/dying issues


REQUIRED KNOWLEDGE, SKILLS & ABILITIES:


Must possess the ability to work as a team member
Must possess the knowledge and commitment to hospice philosophy
Must demonstrate good verbal and written communication, and organization skills
Must possess an understanding of grief response and experience with counseling individuals and/or in groups regarding loss
Must be able to travel often without notice
Must be flexible in work hours


WORK ENVIRONMENT:


Must comply with company policies, procedures and processes in addition to Hospice and Homecare rules and regulations
Must comply with harassment and discrimination policies
Must foster good working relationships
Must be dependable and flexible in work schedule
Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
Must be willing to submit to a criminal background check as well as a drug/alcohol screening upon hire and periodically thereafter



PHYSICAL ENVIRONMENT:


May have to travel in inclement weather
May be required to stand and sit for long periods
Must be able to hear and see with or without aid
Must be able to read and follow instructions as required
