INTERNATIONAL COMMUNITY HEALTH SERVICES seeks a Behavioral Health Specialist II in Shoreline, WA to Provide brief, integrated behavioral health services within a primary care setting. Utilize patient health data and proactively engage patient panels to achieve population defined behavioral health outcome. Conduct evaluations to help patients establish mental health evidence or a combination of mental health and medical evidence of a disability. Screen, identify, and manage patients who present difficulties with mental illness, substance use, and other social determinants of health risk factors that impact patient’s behavioral health. Assess patients through obtaining psychosocial histories to identify and justify a behavioral health related diagnosis. Develop and implement brief treatment plans that use evidence-based treatment modalities that reflect the patient’s diagnosis, modify treatment and plans as clinically indicated. Support psychotropic medication management prescribed by primary care clinicians with a focus on treatment adherence, attention to side effects, and effectiveness of treatment. Facilitate patient engagement and follow-up in health care. Conduct scheduled consultation with consulting psychiatrists and communicate resulting treatment recommendations to patient’s physician or other team members. Track patient treatment, follow up, and clinical outcomes as required. Accept internal referrals and make external behavioral health referrals when appropriate and serve as a resource to the health care team. Consult, collaborate, and coordinate behavioral health services internally with the patient’s care team and externally with care providers and natural supports as clinically indicated.

Minimum Requirements: Masters in Social Work, Psychology, Counseling or related field of study including Christian Counseling AND 24 months experience as Counselor, Therapist, or related position working with patients with mental health, substance abuse and/or disability issues.

Wage Offered: $74,984 to $109,784 per year
