Job Summary

Works in conjunction with the Director of Behavioral Health and the Clinic Manager to actively and consistently contribute to the clinic operations and communications; fully integrate primary care with mental healthcare; behaves in a manner consistent with the mission and objectives of OCHC and performs duties as requested.

Education & Training

Must pass annual competencies
Active professional development
Satisfactory maintenance of continuing education standards as established by the Division of Professional Registration of Missouri

Licensure & Certifications

Current licensure of one of the following:
Licensed Clinical Social Worker
Licensed Master Social Work
Provisionally Licensed Professional Counselor
Licensed Professional Counselor
Family and Friends CPR within 6 months of employment

Experience

One-year experience preferred

Job Responsibilities
  • Performs screenings/evaluations of individuals for mental health and substance abuse disorders.
  1. Performs brief interventions for individuals with behavioral health problems.
  2. Provides behavioral supports to assist individuals in improving health status and managing chronic illnesses as needed
  3. Meets regularly with the primary care team to plan care and discuss cases and exchanges appropriate information with team members in an informal curbside manner as part of the daily routine of the clinic.
  •  Integrates with primary care providers.
  1. Supports primary care physician/teams in identifying and behaviorally intervening with patients who could benefit from behavioral interventions.
  2. Participant in front line interventions with first looking to manage behavioral health needs within primary care practice.

  • Identifies problem behavior, discusses impact of intervention, and decides what to change.
  • Implements specific and goal directed interventions.

  5. Provides handouts explaining appropriate information.

  • Tailors education to each specific patient and issue.
  • Consistently provides feedback to primary care physician.
  • Communicates in a brief, concise, and brief manner.
  • Maintains focus on referral question.
  • Provides description of action plan.
  1. A. Provides plan for follow-up care.
  2. Transitions patients to traditional behavioral health counseling as needed.
  3. Uses correct visit templates when documenting, differentiates visits between BHC and counseling consistently.
  4.  Dismisses each patient by walking them to the front office communicating with office staff what the patient needs to be scheduled for and if it is a BHC appointment what number that session will be.
  5. Ensures Medicaid prior authorizations are obtained prior to 5th patient visit.
  6. Provides comprehensive mental health care to patients per OCHC procedure and policy and collaborative practice agreement/protocol, utilizing supplies and procedures appropriate to ages of the patients serviced including child, adolescent, adult, and geriatric patient population for the subsequent duties.
  • Obtains and documents all relevant health history.
  • Performs mental health examination and documents relevant findings.
  • Develops and implements a plan of care, with the patient and appropriate others, which may include additional testing, education, counseling, health promotion, referral, and follow-up, and documents in the patient record.
  • Educates and counsels individuals and/or their families on health, health promotion, and illness, based on identified mental needs.
  • Provides mental care related consultation to and collaborates with community agencies and organizations.
  • Documentation is completed and signed within 72 hours after patient has been seen.
  • Provides correct documentation, diagnoses codes and correct template to ensure correct coding/billing occurs.
  • Corrections from coding are completed in a timely manner.
