This position is responsible for cultivating and maintaining strategic relationships and referral stream pathways across the designated service county or counties, connecting community partners with ACC’s full menu of Behavioral Health and Wraparound services. The role bridges clinical programs and community systems through proactive engagement, education, and collaboration.

Community Outreach & Relationship Development


Establish, develop, and maintain working relationships with referral sources throughout the designated county or counties, including courts and probation offices, school districts (public and charter, K-12, Higher-Education, etc.), faith centers, community centers, housing programs, hospitals, and other healthcare and social service providers.
Serve as a knowledgeable ambassador for ACC’s behavioral health services, communicating the full scope of program offerings to prospective referral partners.
Conduct regular outreach visits and attend community events, coalition meetings, and partner forums to raise awareness of ACC services and build collaborative networks.
Identify gaps in current referral streams and develop targeted strategies to cultivate new referral sources within the community.


Referral & Intake Coordination


Manage and facilitate referral processes from point of contact through intake, ensuring a smooth and timely transition for individuals entering ACC programs.
Track referral volume, sources, and conversion rates; enter data in Birdseye Database and report to the Director of Behavioral Health on a regular basis.
Serve as the primary point of contact for referral partners, addressing questions and ensuring an efficient and welcoming referral experience.


Partnership & Program Education


Develop and deliver informational presentations and educational materials on ACC’s programs tailored to specific partner audiences (e.g., court staff, school counselors, hospital social workers).
Participate in cross-sector initiatives, task forces, and collaborative tables relevant to behavioral health access and care coordination in the community.
Maintain up-to-date knowledge of local behavioral health resources, payer and insurance requirements, and community needs to inform outreach strategy.
Conduct pre/post surveys of events as well and community needs assessments to aide with program and service development.


Case Management & Client Support


Carry an active case management caseload as assigned, providing coordination, advocacy, and supportive services to individuals engaged in behavioral health programming.
Conduct assessments, develop individualized service plans, and coordinate linkages to community resources including housing, transportation, social services, and peer supports.
Monitor client progress, document services in the electronic health record (EHR) in accordance with regulatory and organizational standards, and communicate updates to the care team.
Provide crisis support and warm handoffs as needed, connecting individuals to appropriate levels of care.
Other duties as assigned based on Department/Organization needs.

QualificationsRequired


Bachelor’s degree in social work, psychology, counseling, public health, or a related human services field.
Minimum 2 years of experience in behavioral health, social services, or community health outreach.
Demonstrated knowledge of behavioral health conditions and community resources.
Strong interpersonal skills and ability to establish rapport across diverse community stakeholders.
Valid driver’s license and reliable transportation; position requires regular county-wide travel across one or more of Oakland, Wayne, and Macomb Counties.
Proficiency with EHR systems and standard office software.


Preferred


Experience working with justice-involved individuals, school-based programs, or hospital and emergency department settings.
Bilingual fluency (Arabic or other languages reflective of the local population served).
Experience with Medicaid and managed care referral and authorization processes.
