Community Health Care is looking for a full-time (40 hours per week) Clinical Therapist (LPCC or LISW) to split time in our Manchester and Norton offices. This Therapist must have a passion for helping others by providing behavioral health services in an integrated setting with multi-disciplinary care teams in a primary care setting. Both offices are energetic, team oriented, and dedicated to providing excellent patient-centered care.  Responsibilities:

· Work within a care team while providing direct counseling services to the patient

· Assess patients for common mental health disorders using evidence-based tools

· Provide brief behavioral interventions using evidence-based techniques such as cognitive behavioral therapy, problem-solving treatment, motivational interviewing, or other treatments as appropriate

· Organize patient outreach to facilitate follow-up care or services

Requirements

· LPCC, LPCC-S, LISW or LISW-S

· Comprehensive knowledge of area resources

· Ability to effectively engage patients in a therapeutic relationship

· Strong verbal and written communication skills including the ability to work with patients by   telephone and virtual visits, as well as in person
