Salary: Starting at $60000 / annually – depending on experience / education

 

Job Summary

Serves as a vital bridge connecting outpatient clinical teams, social service networks, and broader community safety nets to support children, adults, and entire family systems. Operating within an outpatient mental health setting, this role conducts comprehensive social needs assessments, navigates complex public and private benefits (including Medicaid, Medicare, and commercial insurance), and manages active, ongoing discharge planning from intake through program completion. This is a non-supervisory role. 


Responsibilities

  • Formulates and executes comprehensive, individualized discharge and transition plans. Identifies required aftercare services to prevent readmission and ensure long-term stability across all age demographics.
  • Serves as the primary bridge between outpatient mental health services, adult and child support networks, and welfare systems.
  • Assesses insurance coverage across all lifespans (Medicaid, Managed Care Organizations, Medicare, Commercial), assists clients and families with benefit enrollment and re-certifications, and resolves coverage barriers.
  • Maintains up-to-date knowledge of diverse community support systems and social determinants of health.
  • Conducts comprehensive social needs assessments for individual adults, youth, and family units, identifies barrier-to-care risks, and establishes measurable social service goals.
  • Identifies behavioral health crises or environmental safety concerns across the family system, coordinates with emergency services and clinical teams for stabilization, and facilitates urgent psychiatric or higher-level-of-care referrals when needed.
  • Maintains timely, high-quality documentation in the Electronic Health Record (EHR).
  • Provides comprehensive service plans and monitors social service goals in conjunction with the treatment team.
  • Participates in supervision.
  • Performs the skills of CPR, the use of the AED, and First Aid on a resident in need, without delay.
  • Performs additional job duties as needed.

Qualifications

  • Bachelor’s degree in Social Work (BSW), Psychology, Human Services, or a related behavioral science field required (Master’s degree / MSW preferred).
  • Minimum of 2 years of case management experience within a behavioral health or outpatient mental health setting, with demonstrated experience serving adults, children, and family units.
  • Direct experience working within or alongside adult and child welfare systems preferred.
  • Effective communication and interpersonal skills.
  • Proficiency in navigating Medicaid, Medicare, Commercial MCOs, SSI/SSDI, SNAP, and regional social safety net programs.
  • Must have own transportation, possess a current/valid Driver’s License, an acceptable Motor Vehicle Record (MVR) as mandated by Woods’ insurance company. Local travel is required.
  • Must be able to lift a minimum of 50 pounds, be agile, physically pursue clients on foot, physically restrain, bend, squat, stretch, reach, push up and stand for several hours at a time.   
  • Must obtain and maintain an active CPR and First Aid Certification.
