JOB SUMMARY:

 

Under the direction of the ECM Program Director, the Behavioral Health Care Manager provides comprehensive, member-centered care management services to individuals enrolled in California’s Enhanced Care Management (ECM) program. This role integrates behavioral health expertise with whole-person care coordination, including outreach, engagement, assessment, care planning, and coordination with healthcare and social service providers.

The position operates within a Community-Based Care Management Entity (CB-CME) and collaborates across interdisciplinary teams to improve health outcomes, address social determinants of health, and ensure timely access to appropriate services.

Requirements

MINIMUM & PREFERRED QUALIFICATIONS:


Education/Training

Minimum: Master’s degree in Social Work, Marriage & Family Therapy, Counseling, Psychology, or related field.

Preferred: Licensed Clinical Social Worker (LCSW) or License Marriage & Family Therapist (LMFT).


Experience 

Minimum: Three (3) years of experience in behavioral health services within a healthcare clinic, psychiatric hospital medical facility, or community-based setting.


Preferred: Experience with Medi-Cal managed care and Enhanced Care Management (ECM programs. Experience in care coordination, discharge planning, or integrated care models. Experience using Motivational Interviewing or similar techniques.

Any combination of educational and work experience that would be equivalent to the stated minimum requirements would qualify for consideration of this position.


Certification(s)

Maintains a valid California Driver’s License and current automobile insurance when using a personal vehicle for Company business.

· Bilingual (English/Spanish) preferred 

· Ability to work independently within a collaborative, interdisciplinary team 

· Sound judgment and decision-making skills in complex situations 

· Adaptability to evolving care models, systems, and ambiguity

     

PHYSICAL, MENTAL & ENVIRONMENTAL REQUIREMENTS:

The physical, mental, and environmental demands described here are representative of those required to successfully perform the essential functions of this position. Primarily field-based role requiring regular travel between Company locations and other business sites as required; including standing, walking, and driving. Involves interacting with Members across home, clinic, and community settings, using standard technology, and managing multiple tasks independently in a dynamic environment. 


PAY RANGE

$90,000 - $95,000 / annually
