Overview

The Behavioral Health & Addiction Medicine Case Manager is responsible for coordinating care and supporting patients with behavioral health and substance use disorders throughout their treatment journey. This position serves as a key member of the interdisciplinary care team, helping patients access treatment, navigate healthcare systems, address social determinants of health, and achieve treatment goals.The Case Manager works closely with behavioral health clinicians, addiction counselors, psychiatric providers, medical providers, community agencies, and family supports toimprove patient engagement, retention, and overall health outcomes.

Responsibilities

Patient Care Coordination• Coordinate behavioral health and substance use disorder services across multiple providers and settings.• Assist patients in navigating healthcare, behavioral health, social service, and recovery support systems.• Develop individualized care plans based on patient needs, goals, and barriers to treatment.• Facilitate communication among treatment providers and community partners to ensure continuity of care.• Participate in interdisciplinary case conferences and treatment team meetings.Patient Engagement & Retention• Conduct outreach to patients who miss appointments or are at risk of disengaging from treatment.• Assist patients in overcoming barriers to care, including transportation, housing, insurance, childcare, food insecurity, and financial challenges.• Support patient adherence to treatment recommendations and recovery goals.• Maintain ongoing contact with high-risk and high-need patients to encourage engagement and retention in services.

Resource Coordination & Advocacy• Connect patients with community resources, benefits, and support services.• Assist patients with applications for insurance, public assistance programs, housing resources, transportation services, and other community supports.• Advocate for patient needs with healthcare providers, schools, employers, legal systems, and community agencies when appropriate.• Support patients in accessing higher levels of care or transitioning between levels of care.Behavioral Health & Addiction Support

• Provide recovery-oriented support and education to individuals with mental health and substance use disorders.• Reinforce treatment goals established by the clinical team.• Assist with referrals to detoxification programs, residential treatment programs, peer recovery services, and community-based behavioral health services as needed.• Support patients with co-occurring mental health and substance use disorders.Documentation & Compliance

• Maintain timely and accurate documentation in the electronic health record.• Track patient outcomes, referrals, engagement activities, and care coordination efforts.• Ensure compliance with HIPAA, confidentiality requirements, and organizational policies.• Participate in quality improvement initiatives and program evaluations.

Community Partnerships• Develop and maintain relationships with community organizations and service providers.• Collaborate with hospitals, behavioral health providers, social service agencies, schools, housing programs, and recovery support organizations.• Assist with referral development and community outreach activities.

Qualifications

Required• Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Public Health, or a related field.• Minimum of two years of experience in behavioral health, substance use treatment, care management, social services, or community-based services.• Knowledge of community resources and social service systems.• Strong organizational, communication, and problem-solving skills.• Experience with electronic health records and care coordination documentation.Preferred• Certified Case Manager (CCM), Certified Recovery Support Practitioner (CRSP), or related credential preferred.• Experience working in an FQHC, behavioral health, addiction medicine, or integrated healthcare setting.• Experience working with individuals with co-occurring mental health and substance use disorders.• Knowledge of Medicaid and managed care systems.• Bilingual English/Spanish preferred.Core Competencies• Patient engagement and retention• Care coordination• Community resource navigation• Crisis intervention and de-escalation• Recovery-oriented care• Interdisciplinary collaboration• Documentation and compliance• Cultural competence• Advocacy and problem solving.
