Position Summary:
The Home Care Manager is responsible for the development and implementation of home care services for program participants. Provides education and teaching to participants, families, and caregivers at the direction of the Interdisciplinary Team (IDT), PACE Center Director, and AVP of PACE Operations. Responsible for oversight of all clinician visits performed in the home setting including facilities, to ensure efficient, high-quality care and patient experience. Coordinates and ensures delivery of all home-based services, equipment, and supplies.
Position Qualifications:
- Is a graduate of an accredited school of professional (registered) nursing.
- Is currently licensed as an RN through the State Board of Nursing.
- Associate’s degree in Nursing required; Bachelor’s Degree preferred.
- At least two years of general nursing experience is required with experience in hospice, home health, medical, surgical, or critical care experience preferred.
- At least one year of supervisory experience required.
- Must have at least one year of experience working with the frail and elderly.
- Food handling permit required, to be completed upon hire.
- Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
- Possesses and maintains current CPR certification.
- Have satisfactory references from nursing school, previous (or current) employers and/or professional peers.
- Have excellent observation skills, good nursing judgment and communication skills. Ability to provide good written documentation in a timely manner.
- Is self-directed with the ability to work with little supervision.
- Is flexible and cooperative in fulfilling role obligation and has the ability to work within an interdisciplinary team.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
- Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
