Scope of Position: The Assistant Director will serve a critical leadership role on one of the Governor’s newly launched Critical Time Intervention (CTI) teams, delivering comprehensive, community-based support to individuals navigating pivotal life transitions. As part of a multidisciplinary team that includes a Director, clinicians, care managers, a registered nurse, and a peer specialist, the Assistant Director will contribute to both program oversight and direct service coordination, ensuring high-quality implementation of the evidence-based CTI model. This role combines staff supervision, operational management, and active community engagement, including outreach to individuals in higher levels of care and coordination of services before and after discharge, with a strong emphasis on addressing social determinants of health and strengthening long-term support systems. The Assistant Director will help guide participants through a structured nine-month intervention designed to promote stability, independence, and sustained well-being in the community. This position requires a hands-on, field-based approach, participation in on-call coverage, and flexibility in scheduling, offering a meaningful opportunity for a clinician to drive innovative change and improve outcomes for vulnerable populations across New York City.
Essential Position Functions:
In collaboration with the Program Director, provide clinical and operational leadership to the CTI team.
Conduct persistent and assertive outreach and engagement using strength-based approaches, beginning during inpatient hospital admissions or emergency department visits.
Partner and collaborate with local hospitals, the Department of Homeless Services, and family members/caregivers to identify individuals most in need of outreach and care.
Continuously assess the health and social needs of participants through CTI’s conversational and observational methods, as well as formalized risk assessment tools for those identified as high risk.
Participate in hospital discharge planning meetings to identify the most appropriate community resources for returning participants.
Provide short-term therapeutic counseling and support to participants pre- and post-discharge.
Collect and report data as required, and work with the team leader, data analyst, and other CTI teams to use data to inform care delivery.
Work with participants and their community providers to resolve clinical issues impacting the participant’s ability to manage and retain supportive services.
Foster relationships with community providers to ensure participants are connected to appropriate services as they transition back into the community.
Provide appointment navigation, including accompaniment, re-engagement in community care, and addressing barriers to care.
Participate in after-hours (24-hour) coverage on a rotating basis.
Facilitate crisis interventions, referrals, and hospitalizations as appropriate.
Review documentation and conduct comprehensive psychosocial assessments to determine medical, psychiatric, housing, and other social needs.
Obtain historical and collateral information from multiple sources to support participants’ behavioral and physical health needs.
Monitor, evaluate, and record participant progress with respect to care plan goals.
Attend and actively participate in team meetings and supervisory sessions.
Perform other related duties as assigned, which may include completing HRA 2010e applications, assisting with housing applications, ensuring participants’ immediate needs are met (including clothing, shelter, and food), and maintaining strong community linkages and supports.
Qualifications
Licensed master’s degree or higher in Social Work, Mental Health Counseling, Nursing, or Psychology.
Experience working with individuals with SMI and co-occurring SUD.
Interest in developing management skills.Knowledge of community resources is a plus.Knowledge of counseling principles and methods for mental illness and substance use disorders.
Knowledge of treatment, rehabilitation, and community support programs as they relate to recipients/residents, families, and staff.Knowledge of techniques for identifying, assessing, and preventing potentially violent behavior, including crisis management and de-escalation techniques.
Ability to develop, evaluate, implement, and modify treatment interventions to meet the needs of individual recipients.
Ability to prepare accurate and timely reports.
Computer proficiency in health information technology and Microsoft applications such as Word, Excel, and PowerPoint.
