Job Summary
We are seeking a Assertive Community Treatment Program Supervisor to join our team! As the ACT Program Supervisor, it will be your responsibility to identify gaps and areas of improvement within our community/ team, and create a plan of action to solve these problems. This can include talking with individuals in need, working with other employees to find solutions to issues, and overall contributing to the community as a whole. The ideal candidate has a compassionate attitude, experience working with people in need, and strong problem-solving skills. 

Responsibilities: The Program Supervisor will/is
  • Oversee all aspects of team operations and shall provide direct services to individuals in the community.
  • Responsible for overseeing the day-to-day operation of the ACT program.
  • Ensure all members of the team are performing their job duties.
  • Conduct an initial assessment consistent with the components required in the Comprehensive Needs Assessment, documenting the individual's diagnoses and describing how service needs match the level of care criteria.
  • Responsible for completing service request authorizations and other clinical documents as required.
  • Direct and authorize the Individualized Service Plan.
  • Complete the daily log and attend all required ACT team meetings.
  • Responsible for reporting all Serious Incidents and filing CHRIS reports as needed.
  • May also provide individual, family or group therapy as needed.
  • Responsible for ensuring all of their personnel documents are accurate and up to date and immediately notifying Human Resources of any changes that could result in their inability to perform their job functions.
Qualifications
  • 3 years of experience in preferred field 
  • Bachelor’s or higher in social work or equivalent field desired
  • LPC or LPC-R
  • LCSW or LCSW-S
  • Experience with social work desired
  • Experience in a people-facing role
  • Strong communication and interpersonal skills 
  • Experience with basic computer programs, such as Microsoft Office suite, and Google suites
