Care Coordinator promotes and supports the objectives of Aging Servics and performs all defined services and other related duties in accordance with the mission of Aging Services.

Responsibilities

Completes admissions as well as discharge planning with the interdisciplinary team

Completes initial intake assessments and ongoing physical and emotional health assessments as needed

Completes and updates individual care plans

Participates in Interdisciplinary meetings and coordinates care with other contracted health providers including Managed Care Organizations.

Provides 1: 1, small or large group therapeutic recreation

Supports participants with personal needs or provides direct care when needed.

Provide assessment and interventions to participants to obtain services and complete reassessments as required.

Assists with medication administration.

May drive participants to and from the center.

Completes documentation per established governing body.

Completes administrative tasks including but not limited to filing, answering phones, participant documentation per established governing regulations and policies.

Is knowledgeable of and adheres to HCBS Settings Rule, Participants Rights, and CARF standards.

Maintains confidentiality of participant information.

Qualifications

Bachelor's Degree in In human services related field including psychology, social work, therapeutic recreation, required

Valid Iowa Drivers License, required

Dependent Adult Abuse Training, within 60 days of hire
