This position acts as an advocate for needed community services when the consumer is in need of concrete services. Core elements of this position include: (1) service plan development; (2) linking and coordination services; (3) advocacy and education provided to consumer to assess needed services and obtain concrete needs; (4) re-assessment and follow-up; (5) monitoring, linking, and coordinating of all services provided to the consumer. The person in this position works with a great deal of professional independence under the general guidance and supervision of the Director of Adult Outpatient Counseling.

 

DUTIES AND RESPONSIBILITIES:


Ensure consumers are receiving eligible SSI, Medicaid, and other medical and insurance benefits Assist and educate consumers with necessary concrete services
Support development of coping skills, in community, to enhance quality of living, prevent psychiatric hospitalization and/or re-hospitalization Increase consumer participation in CMH aftercare and support services
Assess, diagnose, and evaluate
Participate in Person Centered Treatment Planning
Act as consumer advocate for all identified needs to support mental health
Enhance continuity of services with multiple providers to ensure health needs, safety issues, legal, DHS, and other pertinent individual issues are addressed
Monthly monitoring of all services provided to consumer
Coordinate care with other internal and community-based treatment services
Responsible for meeting direct service productivity and documentation standards per agency and program expectations
Maintain consumer case record, prepare reports and correspondence with professionalism, in a timely manner
Must meet agency productivity/performance standards
Other tasks and responsibilities, as assigned

MINIMUM QUALIFICATIONS:
Bachelor's degree in social work, Psychology, Counseling, or a related behavioral health field, and a valid State of Michigan professional license in the applicable discipline (e.g., LLBSW, LBSW, LLMSW, LMSW, LLP, LP, LLPC, or LPC), or the ability to be credentialed as a Qualified Mental Health Professional (QMHP).
Current CPR/First Aid Certification, preferred
One year case management working with adults and families with psychiatric disorders
Bilingual proficiency in Spanish preferred

Compensation will be offered based on the candidate's education, experience, and internal equity.
