As an Administrative Case Manager, you’ll play a vital role in connecting individuals to essential community resources and supporting LVF’s mission to improve lives. This is a full-time position based in beautiful Doylestown, PA.

What You Bring

  • Bachelor’s degree in human services, Psychology, Social Work, Sociology, or related field preferred
  • HS Diploma with 12 credits in Social Sciences and 2 years’ experience accepted
  • Strong computer skills: EHR experience preferred
  • Valid Driver’s License and ability to travel within Bucks County
  • Strong verbal and written communication skills
  • Ability to meet the physical demands of the role, with or without accommodation

 

Your Role

  • Provide short-term case management for individuals seeking services at LVF
  • Partner with clients in making connections to community resources including accessing health insurance, housing programs, county assistance benefits, transportation options, and providing support in problem solving benefit issues
  • Travel to other LVF sites to help with Medical Assistance applications
  • Receive referrals and provide immediate follow-up
  • Document all work in our electronic health system
