Position Overview:
The Addiction Recovery and Treatment (ARTS) Care Coordinator provides administrative coordination across the agency's Substance Use Disorder (SUD) continuum of care, including ASAM Level 1 Outpatient Services, ASAM 2.1 Intensive Outpatient Services, ASAM 2.5 Partial Hospitalization Services, ASAM 3.1 Residential Services, and Medication Assisted Treatment (MAT) services.
The position serves as the primary coordinator for monitoring client engagement, service utilization, treatment progression, and compliance with ASAM service intensity recommendations. Responsibilities include tracking required and completed service hours, identifying service gaps, supporting care coordination, promoting engagement and retention, facilitating transitions between levels of care, and helping clients access services needed to meet treatment goals and medical necessity requirements. The position collaborates with clinical staff, peers, case managers, MAT providers, residential staff, and community partners to support continuity of care and successful treatment outcomes.
This position does not have direct supervisory responsibilities but provides coordination and consultation to treatment teams regarding service utilization, client engagement, and continuum of care planning.
Qualifications & Requirements
- Bachelor's degree in human services, social work, counseling, psychology, nursing, rehabilitation, or a related field
- Experience working with individuals with substance use disorders
- Experience providing case management, treatment coordination, or behavioral health services
- License-eligible or Licensed Mental Health Professional (LPC, LCSW, LMFT, or equivalent) preferred
- Medication Administration certification, or ability to obtain, preferred
- Valid Virginia Driver's License required
Preferred Skills & Experience:
- Experience coordinating client care across multiple levels of the ASAM continuum, including outpatient, intensive outpatient, residential, and recovery support services
- Knowledge of substance use disorder treatment, recovery principles, Medication-Assisted Treatment (MAT), and community-based support resources
- Ability to monitor treatment participation, track service utilization, and identify gaps in client engagement and continuity of care
- Experience conducting client outreach, engagement, retention, and follow-up activities to support successful treatment outcomes
- Strong case coordination skills, including facilitating referrals, transitions between levels of care, and access to appropriate services
- Ability to collaborate effectively with multidisciplinary teams, including counselors, case managers, peer recovery specialists, and medical providers
- Experience identifying barriers to treatment participation and developing individualized strategies to support client success and retention
- Strong organizational, communication, and documentation skills, with the ability to manage census data, waitlists, service capacity, and client tracking systems
